Course Description:
This course is designed to educate participants in the importance and necessity of specificity and accuracy in the coding and documentation throughout the rehabilitation episode. Coding of patient diagnoses and co-morbidities to the highest level of specificity is necessary for accuracy with Clinical Categories, and the frequent coding of patient functional task performance is necessary for accurate functional reporting. The benefits of the use of clinical pathways and evidence-based resources and references in the planning and progressing of the individual patient treatment plans will be discussed; as well as documentation strategies to support treatment intensity and duration. New EMR format to facilitate supportive documentation for PDPM will be reviewed and participants will be trained on its use in planning, providing, and documenting patient-centered care. As leaders of the therapy team at their nursing facility, participants will be able to educate and lead their staff in the information and techniques presented.
